Yes, a user has been banned at immortal rank although the user claims to be manual ban. Myself, I have used the hack for over 20 games with some callouts that I am using esp but still no ban so far. I expect my account to either be manual banned or riot detects the cheat when they come to know about this.

You can score more than 6, but if you get a report, you will be watched a lot and suddenly you can get banned, everything is in your hands.

If you don't mind me sharing my experience. There are really no problems when injecting, the esp and radar does unload at times but it updates when the round begins. So far I have only been using one account with my account name and level hidden. I play obviously with it and get called out sometimes but I don't really use it to grind competitive although I don't follow the 6 matches per day. @berkaysensiz may have a different experience
