In my opinion Di Caprio is completly sane, there is nothing wrong with him. Watching for the 2nd or 3rd time, you can see there are too many signs that something is really wrong with the place

This theory is supported by several clues in the film, such as the recurring visions and hallucinations that Teddy experiences, the inconsistencies and anomalies in his memories and the story, and the reveal at the end that Teddy's real name is Andrew Laeddis and he was committed to the hospital after killing his wife in a fit of rage. This theory suggests that the entire plot of the film is a manifestation of Teddy's mental state and the events on the island are a part of his therapy and treatment at the hospital.

Yeah this movie was hella trippy for me, But for me I believe he was completely sane in the last scene and chose to trick the other characters because he didn't want to continue living knowing what he had done. I think this is shown when he asks "Is it better to live as monster, or die as a good man".

I've watched the movie more than once and it's really disturbing, but I don't think the main actor is crazy, but that everything is nothing more than the simplest and purest reality.
