Firstly I want to believe that you have antivirus and firewall turned off, AdBlock as well. When you load the script from the Hack UI Center, a new window opens saying "Waiting for LeagueOfLegends.exe". Start a match and then the script will be injected (in-game match not the lobby)

Disable your adblock, allow windsharp on your pc antivirus (if you have one), then just start a game and open it.
If you did all of the above, try reinstalling it, if still doesn't work, well, you might as well have some corrupt reference assembly, I would check on that as well.
